CLINIC FORMS AND INFORMATION

TWMS staffs a clinic at Elementary/Middle School. The Elementary/Middle school clinic is staffed by a full-time RN.

Information regarding medical forms, student absences due to illness, medications, allergies, immunizations, and health screenings can be found in the Student/Parent Handbook.


Physical Deadline
If your child is in 5th-8th grade and will be participating in Athletics, a Sports Physical is required after April 1st of the previous school year.
